What this is

PEP (post-exposure prophylaxis) is a 28-day course of HIV medicine you take after a possible exposure to stop the virus from taking hold. This pack contains one fixed-dose tablet a day of tenofovir disoproxil fumarate 300mg, lamivudine 300mg and dolutegravir 50mg, often called TLD. It’s the regimen the World Health Organization prefers for PEP, because it works well, is usually easy to tolerate, and HIV rarely develops resistance to it.

When PEP is recommended

PEP is for people who are HIV-negative and had a possible exposure in the last 72 hours, such as:

  • Vaginal or anal sex without a condom, or a condom that broke or slipped
  • Sexual assault
  • Sharing a needle or syringe
  • A needlestick or blood splash at work

It’s not usually needed after oral sex without ejaculation or blood, or after kissing or touching. If you’re unsure, our 2-minute risk checker will tell you honestly.

How to take it

  • Start as soon as you can, ideally within 24 hours and no later than 72 hours after the exposure.
  • Take one tablet once a day, at about the same time each day, for all 28 days.
  • If you miss a dose, take it when you remember. If it’s almost time for the next one, skip the missed dose. Never take two at once.
  • Finishing the full course matters. If you’re thinking of stopping, message us first.

Testing

A baseline HIV test is required before PEP, unless you already know your status. Our rapid HIV test kit is a separate paid add-on, subject to availability: add it to your order and test before your first dose; it takes about 20 minutes. If a kit isn’t available, don’t wait: start PEP and test as soon as you can. Then test again at 6 weeks and at 3 months after the exposure, using a fourth-generation test. A negative result at 3 months is conclusive.

Tell us if any of these apply

  • Antacids, iron, calcium, zinc or multivitamins: take PEP at least 2 hours before them, or 6 hours after.
  • Metformin, rifampicin, or epilepsy medicines like carbamazepine or phenytoin: your dose or regimen may need adjusting.
  • Kidney disease or hepatitis B: we’ll check PEP is safe for you.
  • Pregnant or breastfeeding: PEP can be used safely, and we’ll make sure it’s right for you.

More on this in our guide to PEP medications.

Side effects

Most people take PEP without serious problems. Some feel mild nausea, headache, tiredness or trouble sleeping in the first week or two, and it usually settles. Taking it with food helps nausea, and taking it in the morning helps with sleep. Read our side effects guide for tips and the signs that need urgent care.

Good to know

  • PEP isn’t a cure for HIV, and it isn’t 100% effective, which is why starting early and finishing the course matter so much.
  • If exposures could happen again, PrEP can protect you before the next one. You can usually start it the day after your last PEP dose.
  • If pregnancy is possible, emergency contraception works best as soon as possible, and some options work up to 5 days after sex.
